Hyperbaric Oxygen Therapy Insurance Coverage
Medicare and most major insurance plans cover hyperbaric oxygen therapy (HBOT) for FDA-approved indications including diabetic lower extremity wounds (diabetic foot ulcers) that are Wagner grade III or higher and have failed standard wound care, delayed radiation injury (radiation tissue damage and osteoradionecrosis), compromised skin grafts and flaps, necrotizing soft tissue infections, osteomyelitis refractory to standard treatment, and other approved conditions. Coverage for off-label or non-approved indications varies by plan and may require prior authorization. Our billing team handles all prior authorization requests for HBOT and provides cost estimates before treatment begins.
Wound Care Services Coverage
Most insurance plans cover medically necessary wound care services including physician evaluation and management, sharp debridement (CPT codes 11042-11047), negative pressure wound therapy (wound VAC, CPT codes 97605-97606), cellular tissue products and skin substitutes (CPT codes 15271-15278), advanced wound dressings (alginate, hydrogel, foam, antimicrobial dressings), compression therapy (multi-layer compression systems, Unna boots), platelet-rich plasma (PRP) therapy, ultrasonic mist therapy, and vascular studies (ankle-brachial index, arterial and venous duplex ultrasound). Coverage levels vary by plan type (PPO, HMO, EPO, POS), deductible status, and coinsurance requirements. Some services require prior authorization.
